Click Here to open and fill out the PDF Application.  Once filled out, you MUST return a copy of the completed application along with your payment and the completed credit card authorization form.  You can send the completed application form via fax at (856) 780-5149, via email at Padonaadm@aol.com or send a hard copy to Candace Jones at the address listed below.

2. Click on the appropriate "Pay Now" link below or send check or money order payable to PADONA to Candace Jones, Executive Assistant, PADONA Membership, 1204 Larchmont Place, Mt. Laurel, NJ 08054.  If you submitted your payment via "Pay Now" you still need to submit a completed application form.  To send the completed application, click on the envelope icon.  A message appears asking how you would like to email this document, you MUST click SEND COPY.  This applies to BOTH the new and renewing members.  Failure to include the completed form will delay processing.
